Insurance

Belmont Shore Physical Therapy accepts most PPO insurances, Regal Medical Group (HMO), personal and auto injury cases, workman’s comp plans; and we are Medicare certified. We also offer a discounted fee for our cash-based patients.

As a courtesy for our patients we will contact your insurance company to verify your benefits for our services and submit all claims to your carrier. However, we do encourage you to contact your insurance provider to check on coverage as well.  Co pays, co insurances, and deductible payments are collected during each visit.

 

Initial Visit/Forms

On your first scheduled appointment, please bring:

  • Driver’s license or a valid form of identification
  • Insurance card/information
  • Prescription/Referral letter (if required or available from your doctor)
  • In addition please complete all new patient forms listed below:

Your first visit will involve a detailed discussion about your recent or chronic injury. Your physical therapist will perform a complete examination and evaluation of the injured area and will come up with a personalized treatment plan so you receive the best level of care.

Follow up visits

Each subsequent visit will include one-on-one time with your therapist.  The therapist will re-assess tissue tightness, joint dysfunctions, movement pattern, flexibility, etc. and he/she will keep track of your progress.

An individualized exercise program will also be designed based on your injury and activity level. A team of physical therapy assistant and aids will help you safely complete all exercises which are monitored by your therapist.

We recommend you wear workout attire or something you feel comfortable moving around in.
